Annual Required Forms (formally called Returning Registration) is now open for grades 8-12. The link for the Annual Required Forms is below. The process for completing the Annual Required Forms is the same as last year. Please note that Annual Required Forms will open for students in K-7 grades on August 1st.
Please understand that completing the Annual Required Forms online is MANDATORY. Do not delay completing these once it’s time to do so. If your child’s forms have not been completed in time, they will not receive a class schedule (secondary) or a teacher assignment (elementary). **For those who do not know the ID number, if you had previously signed up on the CPSD app, you should be able to find the ID number associated with your child there. It should appear on the second page of the app with your child's picture. Touch their picture and you will see their student ID number. If you have more than one child in the district, use the ID of your youngest child. The password should be your child's uppercase first initial, lowercase last initial, and their 6-digit date of birth. There are no spaces, commas, or special characters in the password.
